What you get with the Neuropathy Consultation:

COMPLETE WELLNESS EVALUATION & PLAN

• 45 minutes private Zoom video consultation with Doctor--This will include a thorough review of your health history, current medications, supplements, diet, exercise, recent diagnostic tests, illnesses, and your goals. You can ask any questions during this time.
• Neuropathy Scoring—You will be sent a Neuropathy Home Testing Kit ahead of your Scheduled Consultation. The neuropathy scoring is achieved during the consult while the Doctor guides you in testing yourself using the kit. Afterward, the Doctor will give a Report of Findings of the severity of your neuropathy.
• Plan of Action—When the initial consult is complete, the Doctor will make the determination if you qualify and are accepted into our Neuropathy Program.
• If we are able to move forward, a Customized Neuropathy Program of Carewill be designed for your personal needs and discussed during the consultation. You will be emailed a copy of this Program of Care.
