What you get with the Consultation:

COMPLETE WELLNESS EVALUATION & PLAN

• 45 minutes private Zoom video consultation with Doctor --This will include a thorough review of your health history, current medications, supplements, diet, exercise, recent diagnostic tests, illnesses, and your goals. You can ask any questions during this time.
• Hormones Score—Doctor Report of Findings of your score determines the severity of your hormonal issues and if you would be a candidate for our Hormones Program. 
• Additional Questionnaires & At-Home Tests as Needed—Based upon the answers in your Hormones Questionnaire, Doctor may request any additional questionnaires & FREE self-tests you can easily perform at home, and the results will also guide the Doctor in deciding what individualized program you need.
• Plan of Action—When the initial consult is complete, Doctor will make the determination if you qualify and are accepted into our Hormones Program.
• If we are able to move forward, a Customized Hormones Program of Care will be designed for your personal needs and discussed during the consultation. You will be emailed a copy of this Program of Care. This Program of Care will include Two Saliva Tests to be performed at home at two time intervals (1 Initial, 1 at 90 Days). These saliva tests are extremely accurate and vital in determining your personalized Program of Care Protocol.
